Creating Form Templates

How to create and customise form templates for intake, assessments, homework, and reflections in Bravely Connect.

Bravely Connect allows you to create and send digital forms — like intake questionnaires, outcome measures, or therapy homework — to clients through Bravely Home. Forms can be tailored to your practice and assigned manually or automatically.

Forms can be used for a range of purposes, including:

  • Intake forms

  • Session feedback

  • Custom outcome measures

  • Consent forms

  • Reflections or homework

  • Supervision or referral documents

To send a form, you must first create a form template.

Create a New Form Template

  1. Go to Forms in the top menu

  2. Click Setup in the top right

  3. Click + New

  4. Enter a form title and description. The title can be edited each time before sending to clients.

  5. Select a form block to start building your form.

  6. Once built, save your form template. Your form is now ready to send to clients!


Form Blocks (Field Types)

You can build your form using variables and the following form blocks:

  • Autofill* – Automatically populates existing fields in client records, like name or emergency contact

  • Text Description – Add static text or instructions (no response needed)

  • Single-line Text – For short, open responses

  • Multi-line Text – For longer, more detailed answers

  • Radio Buttons – Clients choose one option from a list

  • Checkbox – Clients can select multiple options

  • Date – For selecting a calendar date

  • Signature – For client acknowledgement or consent

📌 Tip: You can preview the form from your client’s perspective at any time by clicking on 'Preview'.


Autofill Form Blocks

One of Bravely Connect's smart features is the Autofill form block. These are typically used in intake forms to collect or update key client information (like contact details or emergency contacts).

Each Autofill field is directly linked to a corresponding field in the client’s profile (e.g. phone number, address, emergency contact). These blocks are helpful when gathering accurate information during onboarding, but they’re best used when you want clients to provide or confirm details you plan to store in Bravely Connect.

⚠️ Use with Care: Any information your client enters into an Autofill field will overwrite the existing data in their client profile.
